Form of report; sending death record to State Marine Board. (1) The State Marine Board shall prepare and make available to the public forms for accident reports required in ORS 830.480. The report shall call for sufficiently detailed information to disclose the cause of an accident, conditions then existing, and the persons and vehicles involved. Every accident report shall be made on a form approved by the board. (2) The State Health Officer shall on or before the 15th day of each month forward to the board a copy of the death record of any persons within the jurisdiction of the State Health Officer who died from a boat accident during the preceding calendar month.
