to 703.310 shall be submitted in writing and under oath to the
Department of Public Safety Standards and Training, accompanied by the proper
fee therefor specified in ORS 703.110.
(2) A fee
submitted to the department may not be refunded. Each application shall be in
such form and require such information as the department prescribes. [1975
