Records of Health Licensing Office. (1) The Health Licensing Office shall keep a record of the office’s proceedings relating to the issuance, refusal, suspension and revocation of authorizations and to the imposition of probation. This record must also contain the name, place of business and date of each authorization issued by the office. (2) The office shall keep a record of all complaints received, including the date of receipt, the name and place of business of each practitioner involved, the name and address of each complainant and the nature of the complaint. (3) The records of the office shall at all reasonable times be open to inspection by the public.
