Statute Text
License and conditional license application; fee.
(1) A person desiring to obtain a
license or conditional license from the State Board of Examiners for
Speech-Language Pathology and Audiology shall make application to the board.
(2) The
application shall be made upon a form and in the manner prescribed by the
board.
(3) The
application required by this section shall be accompanied by a nonrefundable
