Statement of health care provider before treatment by denturist.
Denturists licensed prior to
January 1, 2004, who have not received an oral pathology endorsement from the
State Board of Denture Technology may not treat any person without having first
received a statement, dated within 30 days of the date of treatment and signed
by a dentist, physician, naturopathic physician, physician associate licensed
under ORS 677.505 to 677.525 or nurse practitioner licensed under ORS 678.375
to 678.390, that the persons oral cavity is substantially free from disease
