Form of commitment. The commitment shall be signed by the justice with the name of office of the justice and may be substantially as follows: ______________________________________________________________________________ JUSTICE COURT FOR THE DISTRICT OF ______ State of Oregon, County of ______ IN THE NAME OF THE STATE OF OREGON To the sheriff of the county aforesaid: An order having this day been made by me that A.
