Statute Text
Commemorative Certificate of Stillbirth; rules.
(1) The State Registrar of the
Center for Health Statistics shall establish a Commemorative Certificate of
Stillbirth. The certificate shall be signed by the state registrar.
(2) The state
registrar shall issue a Commemorative Certificate of Stillbirth for a
stillbirth occurring on or after January 1, 1999, upon:
(a) Request of a
genetic parent of the stillborn fetus; and
(b) Payment of
the fee adopted by the state registrar by rule.
(3) The state
registrar shall adopt by rule:
(a) A form for
the certificate;
(b) The type of
information that may be included on the form; and
(c) The fee
required for issuance of the certificate.
(4) A certificate
issued under this section is for commemorative purposes only and has no legal
