University of California, San Francisco Police Department General Orders 10.2 Multiple Arrest Procedures 10.2.7 Field Arrest Form (Issued: 6/25/07) A. The Field Arrest Form is designated to expedite the Field Arrest process as well as allowing any person taken into custody to be tracked through arrest or detention process and identified at a later time. The form will be filled out with a black pen to aid in photography. 1. Only one form needs to be completed per location of arrest. 2. The charges and control numbers may be rotated by attaching a 3” x 5” c ard to the sheet with the appropriate information for the individual arrestee. a. The same case number will be used for all “mass” arrestees from the same location, when possible. b. Arrests made during the event, but not stemming from the “mass” arrest, have individual case numbers for each incident, including multiple defendants.
