University of California, San Francisco Police Department General Orders 8.4 Records: Administration 8.4.11 New Form Development (Revised: 7/18/18) A. The PS D Commander or designee is responsible for the development and modification of all forms used by the Department , as outlined in General Order 2.3.3, “Agency Forms Control.” Input from staff using the form will be obtained prior to any modifications. B. The PSD Comm ander or designee will review existing forms annually to ensure: 1. There is no duplication of information 2. The format is consistent with UCSF Police Department protocol of Records maintenance or data processing requirements. C. Final approval of all ne w forms will be made by the Chief of Police.
