University of California, San Francisco Police Department General Orders 3.41 Performance Evaluation: Utilization 3.41.3 Unsatisfactory Performance (Revised: 8/28/19 ) Employees will be notified in writing by their supervisors if their performance is unsatisfactory. Documentation of a performance issue must be made in a timely manner . Performance issues that occur within 90 days of the end of the annual rating per iod will be documented in the employee’s annual evaluation. The notification should include a description of the unsatisfactory performance, measures that should be taken to improve the employee’s performance and specific requirements and time frames for necessary improvement. Appropriate forms of notification are letters or memoranda. If no improvement is mad e by the end of the rating period, this information should be included in the employee’s performance evaluation report and appropriate corrective and/or disciplinary action taken , in accordance with University policy and the relevant collective bargaining agreement.
