University of California, San Francisco Police Department General Orders 3.40 Performance Evaluation: Management 3.40.4 Review and Appeal Process (Revised : 2/2/11 ) A. If a member believes s/he has received an unfair rating, the member shall immediately discuss his/her concerns with the rater in an attempt to reach a satisfactory resolution. B. If a satisfactory resolution is not reached, the member shall present his/ her concerns in writing to his/her Division Manager within 30 calendar days of the date the evaluation was presented to the employee. C. If still unresolved, the employee must request a review by the Chief of Police. The Chief of Police, whose decision is final, will respond back to the member. D. Personnel may add their comments and/or supportive documents to their performance evaluation within 30 calendar days of date the evaluation was presented to the employee. E. Personnel shall refer to their respective collective bargaining agreements and PPSM agreement grievance procedures relating to individuals receiving a “less than satisfactory” rating.
