3.12 Grievance Procedures
3.12.3 Grievance Information (Revised : 1/10/22 )
Grievances will be submitted in writing, as prescribed in the applicable grievance procedure. At
a minimum, written grievances will include the following information:
A. A statement of the grievance and the facts upon which it is based
B. An account of the specific wrongful act alleged and harm done
C. A statement of the remedy or adjustment sought.
