CATEGORY DATE ADOPTED LAST REVIEW 2 01/24/2011 07/30/24 TUSTIN POLICE DEPARTMENT STANDARD OPERATING PROCEDURES SS705 - Forms 1 SS705 FORMS SS705.1 PURPOSE AND SCOPE The purpose of this procedure is to describe the process for establishing a Forms Committee and following proper guidelines to ensure all forms are current and updated. SS705.1.2 ACCREDITATION STANDARDS This procedure pertains to the following CALEA Standards : 11.4.2 This p rocedure pertains to the following General Orders: N/A SS705.2 PROCEDURE SS705.2.1 FORMS COMMITTEE A Forms Committee comprised by various department personnel has been established , with the Records Supervisor serving as the chairperson for this committee. The chairperson is responsible for overseeing the creation , modi fication, review and approval for all forms used by the department. SS705.2.2 FORMS CREATION AND CHANGE PRO CESS All employees are encouraged to recommend new forms or changes to existing forms when the result of such action will lead to a more efficient or effective manner in which we conduct business. Recommendations are made to the Records Supervisor via the employee’s chain of command. When a new form is proposed, or an existing one needs updating the following protocol is to be followed: a) The form is circulated to concerned or related employees for their input ; b) The c ommittee discuss es the proposed form or recommended changes; c) The form is forwarded for review and approval by the Chief of Police, or their designee via the appropriate chain of command for ; d) Upon approval, the form is forwarded for printing or updating ; e) When the form goes into circulation , a notification indicating the introduction of, or changes to the form will be sent to department personnel ; f) The notification will indicate the destruction of all former versions of the form; g) For control purposes, t he Records Unit will maintain a forms book with a copy of the new or updated form version .
