SS401 – Premise History / Address Alert 1 SS401 PREMISE HISTORY/ADDRESS ALERT
SS401.1 PURPOSE AND SCOPE
The safety of field personnel and the public is of utmost importance in handling calls for service.
Having knowledge of potential dangers associated with a location is vital in keeping with this
goal. This procedure est ablishes guidelines for entering pr emise history/address alerts for
locations in the city.
SS401.1.1 ACCREDITATION STANDARDS
This Procedure pertains to the following CALEA Standards: N/A

SS401.2 PROCEDURE
When officers respond to a business or residence and learn of potential dangers associated
with future responses to the same location, they will fill out a “Premise History” form. This form
is located on the “S” drive. The handling officer should complete this form , however i f due to
injury or any other circumstance where he/she is unable, another officer who responded to the
address should fill out the form as described below.
a) Fill in the name of business (if applicable), the address, along with the related subject’s
name, date of birth and physical description;
b) Complete the narrative section. This is w here the officer will describe the potential threat
and what action if any should be taken. (For example: “Any calls for service at this
residence require a 2 unit response along with a supervisor ” preceded by the reasons
why);
c) Upon complet ion, submit the form to a supervisor for approval;
d) The approved/signed form will then be submitted to a Communications Officer for entry
into the Record s Management/CAD system;
e) The Communications Officer will then file the hard copy of the form.
