Reporting
1022.1 PURPOSE AND SCOPE
The purpose of this policy is to provide guidance regarding the timely reporting of occupational
diseases, mental health issues, and work-related injuries.
1022.1.1 DEFINITIONS
Definitions related to this policy include:
Occupational disease or work-related injury or illness - An injury or illness, including
psychiatric , disease, or mental health issue arising out of employment (Labor Code § 3208; Labor
Code § 3208.3; Labor Code § 3212 et seq.).
When the injury or illness is believed to have been caused or aggravated in the course and scope
of employment, it is also known as a compensable injury or illness, and the employer is found to
be liable for benefits under the provisions of the applicable sections of the California Labor Code
for Worker's Compensation (City of Santa Monica Administrative Instruction III-5-1: Work Related
Injuries and Illnesses).
1022.2 POLICY
The Santa Monica Police Department will address occupational illnesses and work-related injuries
appropriately, and will comply with applicable state workers’ compensation requirements (Labor
Code § 3200 et seq.).
1022.3 RESPONSIBILITIES
1022.3.1 MEMBER RESPONSIBILITIES
Any member sustaining any occupational illness or work-related injury shall report such illness or
injury as soon as practicable, to his/her supervisor, and shall seek medical care when appropriate
(8 CCR 14300.35). In any case, the member shall report the work-related illness or injury to his/
her supervisor no less than one year from the occurrence or knowledge of the injury or illness.
The employee shall complete the City's internal report form RM-WC-01, Employee's Report of
Industrial Injury/Illness.
Any member sustaining a work-related injury or illness that requires relief from duty is required
to be examined and treated by a doctor. The City's Workers' Compensation Unit staff will
authorize and coordinate the member's medical treatment with the exception of any injury or illness
necessitating emergency room treatment.
After a member is treated for any work-related injury or illness, he/she shall report as soon as
practical to his/her supervisor the following:

Reporting - 2(a) Any medical release to return to work with or without restrictions.
(b) Extent and duration of any work restrictions.
Members who miss time from work due to compensable job related injuries or illnesses must
advise their supervisors of their work status after every visit to the treating doctor(s). Prior to
resumption of full duty or temporary modified-duty, members shall submit original work status
notes from the treating doctor(s) to his/her supervisor after every visit. Members provided
temporary modified-duty assignments shall adhere to the Department's Temporary Modified-Duty
Assignment policy.
1022.3.2 SUPERVISOR RESPONSIBILITIES
A supervisor learning of any work-related injury or illness shall:
(a) Ensure the injured member receives medical care as appropriate.
(b) Promptly complete the appropriate forms and forward them though the chain of
command to the Division Commander.
1. For a work-related illness or injury not requiring medical attention, the
Supervisor's Report of Accident Investigation (RM-WC-03) form shall be
completed and signed.
2. For a work-related Illness or injuring requiring medical attention, the Supervisor's
Report of Accident Investigation (RM-WC-03) form and State of California
Employer's Report of Occupational Injury or Illness form shall be completed and
signed.
3. The internal forms, including the Employee's Report of Industrial Injury/Illness
(RM-WC-01) and the Supervisor's Report of Accident Investigation (RM-
WC-03), should be submitted to Risk Management within 24 hours of the
supervisor's date of knowledge of the injury or illness.
4. The internal forms can be located on the City's Intranet through Risk
Management. The employer forms can be located on the State of California
Department of Industrial Relations website.
(c) Every injured member must be provided with a Worker's Compensation Claim Form
(DWC-1) within 24 hours, regardless of the nature of the illness or injury.
Supervisors shall determine whether the Major Incident Notification and Illness and Injury
Prevention policies apply and take additional action as required.
1022.3.3 DIVISION COMMANDER RESPONSIBILITIES
The Division Commander who receives a report of an occupational illness or work-related injury
should review the report for accuracy and determine what additional action should be taken. The
report shall then be forwarded to the Chief of Police, Deputy Chief of Police, and Risk Management
to ensure any required Division of Occupational Health and Safety Administration (Cal/OSHA)
reporting is made as required in the illness and injury prevention plan identified in the Illness and
Injury Prevention Policy.
