Training Page 1 of 2 4/09.0 (REV 11/2020 ) CRIME AND CASUALTY REPORTS The purpose of this order is to establish the procedure s for promptly preparing and processing crime reports and casualty reports relative to incident s occurring at the Academy, EVOC, Range , or any offsite location where In -Service or Academy scheduled training is taking place . All references in this order to “employee (s)” encompa sses employees, instructors, recruits, and students. I. General A. All crimes and injuries involving on-duty employees, instructors, recruits , or students shall be reported on the appropriate report form and prepared in accordance with departmental policies and procedures. B. The preliminary investigation and documentation will be completed immediately after the incident occurs or is reported. Reports will be completed before the investigating officer’s end of shift or at the direction of the Academy Director . C. Original reports will be promptly submitted to the supervisor on duty upon completion. The on -duty supervisor will be responsible for reviewing the report and approving it. II. First Aid or Medical Treatment Only A. First aid or medical treatment is any one -time treatment and any one follow -up visit for the purpose of observation of minor injuries which do not ordinarily require medical care even though the treatment and follow -up may have been provided by a physician or other health care professional. Also, the injured or ill employee must not have lost any workdays other than the day of the injury or onset of illness. III. Serious Injury or Illness A. Serious injury or illness is any incident that results in fatality, loss of consciousness, transfer to another job, restriction of work or motion, and/or lost workday beyond the day of the incident. It includes any incident which requires medical treatment beyond first aid and/or when prescription medication is used beyond a s ingle dose administered on the first medical visit. A gross blood or body fluid exposure is a large amount of blood or body fluid that makes contact with an employee’s intact skin or any amount of blood or body fluid contacting open cuts or sores and/or mucous membrane of an employee. IV. Required Forms for First Aid or Medical Treatment Only Claims 1. The injured or ill employee’s immediate supervisor shall provide the injured/ill employee with the State of California Employee’s Claim for Workers’ Compensat ion Benefits form. a. The employee also may write a Casualty Report to docu ment their involvement in a particular incident. A copy of the Casualty Report shall accompany the claim package. 2. The employee’s immediate supervisor shall complete the appropriate Worker’s Compensation forms within 24 hours of receiving notice of an industrial injury/illness. NOTE: All forms may be accessed on the SSDWeb, Human Resources Links , “Workers’ Comp Info & On Line Forms .” V. Distribution of Required Forms 1. The supervisor shall: a. Ensure the Injury/Illness report package is completed within 24 hours of the date and time the incident was reported by the affected employee . b. Email the completed documents to the Sheriff’s WC/MD Coordinator at medical@sacsheriff.com and the Sacramento County Worker’s Compensation Office at dpsworkscomp@saccounty.net .
