EVIDENCE ENVELOPE
The purpose of this order is to d escribe the procedures used to complete this form.
I. Complete the Evidence Envelope as Follows:
A. Report Number: Sacramento Sheriff’s Report Number.
B. Crime Type: Cite the applicable code section.
C. Beat: Use this area to indicate the district from which the property was
collected.
D. Sub-Beat: Not applicable. E. Boxes: Check appropriate box. F. Day-Date: The day of the week and date the property was coll ected.
G. Time: The time the pr operty was collected.
H. Victim: Name the victim of the crime committed. If more than one victim,
identify the victim and the property connected to the victim(s) . If the
property is connected to more t han one victim, indicate additio nal victim(s)
in the Remarks section.
I. Address: Address of victim. J. Scene Location: Description o f scene location from which the property
was collected. Indicate the addr ess (if there is one), and any other
description that may be applicabl e; i.e., living room, kitchen, sidewalk, etc.
K. Officers: Name of officer(s ) collecting and booking property .
L. Investigator(s): Names of t he Crime Scene Investigator(s) or ID
Technician(s) collect ing and booking property.
M. Remarks: Indicate the item num ber from the Property Receipt/ Report and
any additional information tha t may assist the Property Bureau (owner of
property, additional vic tims/suspects, etc.).

N. Number of Latent Lift Cards Enclosed: Self explanatory.
