Property Bureau Page 1 of 3 60/02.0 (REV 8/86) CONTROLLED SUBSTANCE ENVELOPE The purpose of this order is to d escribe the procedures used to complete this form. I. Use and Packaging A. All narcotics (evidence and c ontraband found) must be enclos ed in or accompanied by a preprinted Cont rolled Substance Envelope which is provided by the Sacrament o County Crime Lab. B. The Kapac plastic envelope process shall be used for all controlled substances with the exception of marijuana unless size is prohi bitive. Controlled substances in liquid form shall first be packaged in a glass container then sealed in a Kapac plastic envelope. The Kapac envelope shall then be placed inside the Controlled Subst ance Envelope i f size permits. C. If the items are too large or bulky to be placed into a Cont rolled Substance Envelope, a bag or box may be us ed with a Contro lled Substance Envelope attached. DO NOT incl ude paraphernalia i n a Controlled Substance Envelope. Note : Requests for analysis of marijuana may be made only by a deputy District Att orney or a narcotics detectiv e. II. The envelope has several ar eas that must be completed by th e officer(s) booking the suspected narcotics. A. Lab No.: Crime Lab use only. B. Property No.: Sacramento Sher iff’s report number is the prop erty number. C. Subject: Include the names of all persons involved or arrest ed for the suspected narcotics. The Crime Lab will refuse evidence in a c ase where the suspect in unknown. D. Sub-Agency: Name of the agency submitting the suspected narc otics to the Crime Lab. This department wil l use the designated SSD in this area. E. Case No.: Report number of agen cy submitting the suspected n arcotics. F. Date Seized: Indicate the date the suspected narcotics were seized. Property Bureau Page 2 of 3 60/02.0 (REV 8/86) G. Violation(s): Cite the appr opriate code and section number. H. Investigating Officer: Name of the officer submitting the su spected narcotics. I. Description of Evidence: 1. Item #: Indicate t he item number fr om the Property Receipt/R eport. 2. Description: Provide the descr iption of the evidence as stat ed on the Property Receipt/Report. 3. Source of Each Item: Location from which items were seized. J. Results of Laboratory Anal ysis: Crime Lab use only. K. Analysis By: Crime Lab use only. L. Chain of Evidence: The submitti ng officer shall fill out the first line. Indicate the name of the submitting offi cer in the “From” section. Indi cate the property locker number in the “T o” section. Complete the line with the date and time the property was booked .
