REQUEST FOR CRIME LAB EXAMINATION
The purpose of this order is to d escribe the procedures to comp lete this form.
I. Purpose
A. The form is to be used primarily to submit property to the C rime Lab for
examination and analysis. However , it also may be used to submi t
evidence to the Sheriff’s Ident ification Bureau. The purpose of the form is
to document the specific exami nation required and to provide a scenario
that will assist the criminalis t in putting the property into t he proper
perspective.
B. When requesting analysis by the Crime Lab, the Request for Crime Lab
Examination form must accompany any and all evidence except that
which is submitted in a Blood Al cohol/Urine Analysis or Control led
Substance Envelope. The failure to provide a completed Request for
Crime Lab Examination form will result in the return of the evidence
without analysis. Note : Request for analysis of marijuana may be made
only by a deputy District Attor ney or a narcotics detective.
II. Complete the form as follows:
A. Lab Case No.: Crime Lab use only. B. Agency: Sacramento Sher iff’s Department – SSD.
C. Agency Case No.: Sacramento Sheriff’s Department report numb er.
D. Offense: Cite the appr opriate code section.
E. Property No.: Sacramento S heriff’s Department report number is the
property number.
F. Date of Offense: Date crime occurred. G. Examination Requested By: Indi cate the name of the person re questing
the examination (pat rol officer, detective, deputy District Att orney)
H. Subject: Include the name of eac h suspect involved or arrest ed in this
case.

I. Victim: Include the name of each victim in this case. J. Name of Investigating Offi cer: Indicate the name and phone n umber of the
detective assigned to this case, if known.
K. Reason for Request: Mark the a ppropriate box. Indicate the d ate the result
is needed, trial date, and name of the deputy District Attorney , if known.
L. Has Any Evidence in the Case Been Previously Submitted for
Examination: Indicate yes and date, no, or unknown.
M. Brief Summary of Circumstanc es: Briefly describe information that might
be helpful to Crime Lab personnel.
N. Evidence Submitted: Itemize using the appropriate number fro m the
Property Receipt/Report. Include a brief description of the ite m and from
whom or where it was seized.
O. Examination Desired: Mark the boxes listed belo w if appropri ate or use
this space for explaining your request.
P. Evidence Received From: Crime Lab use only. Q. The completed form shall be routed as follows:
1. White – Retained by submitti ng agency (include in report pac kage).
2. Yellow – Routed wit h the evidence to Lab (place in property lockers
and attach a “Crime Lab” or “ID” tag to the outside of the lock er).
3. Pink – Included in prosecuti on package when submitted to the DA’s
office (include with orig inal report to Records).
