Property Bureau Page 1 of 6 60/00.0 (REV 2/96) PROPERTY RECEIPT/REPORT FORM The purpose of this order is to d escribe the procedures used to complete the Property Receipt/Report form. I. General The property receipt/report form is to be used as a combination property report and property receipt. It should be used in processing recovered stolen property, evidence, contraband, safekeeping , and found property. Its purp ose shall include the establishment of a chain of e vidence, and provide for the u ltimate disposition of property to the owner. The last copy (goldenrod) shall be us ed as a receipt in all cases when Sheriff’s personne l assume possession of propert y and/or contraband. II. Front Side of Form A. Report number. Indicate the re port number in this area. The report number is required on all reports. B. Seized by Search Warrant No . The warrant number shall be rec orded in the space provided if the proper ty was seized pursuant to a sea rch warrant. A copy of the Return of Search Warrant when signed by a judge shall be immediately sent to t he Property Bureau. When marking this box, either the “Evidence” or “Recover ed Stolen” box must be marked (see Sample 2). C. Asset Forfeiture D. Number of Suspects E. Recovered Stolen (Crime Repor t Optional). Property known to the officer to be stolen and the victim ident ified. If this box is marked, the property report may be used as a stand al one report. However, when using as a stand alone report, the body of t he property report shall conta in an explanation of the circumstances regarding the recovery of the property (see Sample 1). F. Evidence (Crime Report Requir ed). Property of evidential val ue in a case in which an arrest or crime r eport has been made (see Sample 2) . Property Bureau Page 2 of 6 60/00.0 (REV 2/96) G. Contraband (No Crime Repor t Required). Property that is ille gal to own or possess and that will not be used as evidence. An explanation o f how this property was obtained shall be included in the body of the prop erty report (see Sample 3). H. Safekeeping (Crime Report Optional) 1. Property that has been accepted or taken by an officer into temporary custody for a known owner. This property has no evidential value. If this box is marked, the property report ma y be used as a stand alone report. However, when using as a stand alone report, the body of the proper ty report shall contain an explanation of the circumstanc es regarding the seizure of the property. 2. This category is applicable when dangerous weapons are taken into custody to prevent a crime ; i.e., removing a weapon from a residence which is the scene of a domestic quarrel (Section 12028.5 PC). A crime report is op tional, however, if a crime re port is not completed, a short stateme nt describing the circumstance s that necessitated removing t he property shall be included in th e body of the property receipt/report (see Sample 4). 3. Weapons booked under Section 12028. 5 PC are releasable after 48 hours. The retention period for all other types of safekeepi ng bookings is 30 days (G eneral Order 22/06.0). I. Found (No Crime Report Requir ed). Property which has been fo und by a citizen or an officer with no k nown owners. This property has n o evidential value (see Sample 5). J. Reporting Date, Day and Time K. Connect Up Reports. Indicate additional report numbers that are related to this report. L. Authority and Secti on. Cite the appropria te code section and number. M. Victim. Record the complete name, telephone number, address (including zip code), date of birth, sex, race of victim. N. Suspect. Record the complete name, telephone number, address (including zip code), date of b irth, age, sex, and race of susp ect. Property Bureau Page 3 of 6 60/00.0 (REV 2/96) O. Item No. 1. List each item separately in numerical sequence. If addition al property reports are needed to compl ete the inventory, continue with the numerical sequence on a separate page; i.e., Page 1, items 1 through 10; Page 2, items 11 through 20 (see Sample 2). 2. If a large quantity of items are being booked and more than one officer is required to inventor y the property, assign numerical sequences to each officer; i.e., f irst officer uses numbers 1 t hrough 49; second officer uses numbers 50 through 99, etc. Note unused numbers from each numerical gr ouping on the property report. 3. In a homicide inve stigation, the ID Bur eau issues all item n umbers for property that is booked. Pri or to completing a property receipt/report, contact the ID Bur eau for the correct item numb er(s) to be used. P. Quantity. List the actual number of items seized. Q. Name of Item.
