Logbooks
This Order defines the policy and procedures for maintaining lo gbooks within the Main
Jail and RCCC .
I. Policy Statement
Correctional facility staff shall maintain logbooks in order to document compliance with
applicable laws, regulations and policies, and keep an accurate record of events at the
Main Jail and RCCC . Supervisors are to ensure such compliance by their review an d
signature. II. Logbook Locations
A. Personnel assigned to each o f the following workstations sha ll maintain a
logbook:
1. Main Jail :
a. Central Control b. Court Control c. Public Counter d. Male and Female Intake e. Recreation Specialists f. Medical and Psychiatric Officer g. Housing Control Rooms (Floors Two through Eight)
h. Male and Female Property
i. Kitchen j. Laundry k. Release
2. RCCC :
a. Gatehouse b. Booking c. Roger Bauman Facility d. Sandra Larson Facility e. Honor Facility f. A Barracks g. B Barracks h. J Barracks

III. Procedures
A. Each workstation that is equi pped with a log book shall also have an
inkpad and a date stamp.
B. All logbooks are official doc uments and shall be treated acc ordingly.
C. Entries into logbooks sha ll be accurate and legible.
1. Errors shall be lined through so that the original entry is still legible.
2. The person making the error will initial and place his/her b adge
number beside the error.
3. The corrected information will then be added.
D. Incidents and information repor ts shall not be recorded in t heir entirety. A
summation will be entered with a refe rence to the actual report .
1. Staff shall complete an “Inmat e/Incident Report” (PF-10) and an
Incident/Information or Cri me Report as required.
IV. Contents
A. Logbooks shall contain entries detailing the event and activ ities of the unit.
At a minimum, the following in formation shall be included:
1. Use of force
2. Placement of inmate in restr aints (other than simply handcuf f/chain
for security transportation/movement)
3. Counts 4. Housing unit checks 5. Housing unit activities: day room (access to phone and showe rs);
outdoor recreation (or reason it was not provided); classes
6. Sick call (nurse, doctor, dental, etc.)
7. Pill call
8. Meals
9. Clothing and linen exchanges
10. Inmate movements (new inmat es being housed, inmates being
transferred or released, inmat es to court, inmates out to
appointments)
11. Commissary
12. Isolation of inmates in holding/safety cells

MJ/RCCC 13. Unusual occurrences (fights, e xtractions, injured inmates o r
personnel, etc.)
14. Shakedowns and inspections
15. Service of process on inmates
16. Delivery of inc oming legal mail
17. Receipt of outgoing legal mail
18. Presence/absence and condition of unit equipment, keys, etc .
B. Personnel shall refer to t he appropriate Operations Order fo r the
appropriate information to be included in the logbook entry.
C. Supervisors’ Signature
1. The Supervisor shall review then sign the logbook to ensure the
entries are accurate and complete while programs and services a re
provided and proper ly documented. For example: outdoor
recreation, housing unit checks , clothing exchange, etc.
2. Each shift supervisor shall sign the logbook at least twice a shift for
the floor or area assigned to h im/her. The signatures shall be at
least six (6) hours apart.
D. Watch Commander’s Signature
1. The Watch Commander or acting Watch Commander shall review
and sign workstation logbooks to e nsure the entries are accurat e
and in compliance with Departmen t policy at least once per shif t.
V. Completed logbooks
A. Completed logbooks may be kept at the workstation for a peri od not to
exceed seven (7) days.
B. The Unit Officer shall deliver the completed logbook to the Main Jail
Archives office or RCCC Administration Arch ives/Server Room.
1. Logbooks shall be maintained in A rchives for tw o (2) years.
2. Logbooks shall be delivered to the Sheriff’s Department Ware house
after two years of s torage in Archives.
