Page 1 of 2 18/01 (REV 1/89) Alterations/Construction or Maintenance The purpose of this General Order is to outline procedures for requesting building maintenance, alterations and/or improvements. I. Alterations and Construction (Form 330) – Major Building alt eration/improvement (construction) should be proce ssed in the following manner: A. Division commanders will submit alterati on/improvement reque sts (Form 330, [Rev 2/87]) for desired proj ects to their respective Chief Deputy in accordance with the dat e specified in the fi scal year budget sc hedule instructions. The Form 330 shoul d be prepared and in cluded with budget requests as outlined in the curre nt budget operations manual. E mergency projects, especially those affect ing health and safety, can be submitted at any time and should include pro per justification. The Form 330 should be completed per instructions cont ained thereon and the block nota ted “Signature of Department Head” is for the signature of the Sher iff or his designated representative. B. The request will be reviewed by the respective Chief Deputy for approval or disapproval. Approved requests will be forwarded to the Depa rtment head for signature and then to the facilities coordinator who w ill consolidate the requests for prio ritizing no later than March 1 of each year. C. The facilities coordinator will review and coordinate all alteration/improvement requests and submit them to the Chief De puty for Special Services who will presen t them to the Sheriff’s Executi ve Staff for prioritizing, and then return them to the facilities coordinato r. D. The facilities coordinator will submit the approved and prio ritized alteration/improvement requests to the ACO Manager, Administrat ion and Finance Agency. E. All inquiries relative to the status of approved projects sh ould be made through the facilities coordinator. II. Maintenance Requests (Form AC 269) A. Maintenance requests are for minor repairs that do not alter or change the facility, such as repair of a door, changing a lock, repairing an electrical outlet, replacing broken floor titl e, painting, etc. With the exception of the Page 2 of 2 18/01 (REV 1/89) Main Jail, Rio Cosumnes Correct ional Center, Work Release Faci lity, and leased facilities who handle build ing maintenance requests in a ccordance with their established procedur es, all requests for building ma intenance projects within the Sheriff’s adm inistration building should be made in writing to the facilities coordi nator, Administrative Division. For emergency items which do not require a m aintenance requisition form, such as elevator problems, leaking fauc ets, or restroom flooding, the M &O Division can be contacted dire ctly by dialing 62121.
