OPERATIONS ORDER
Reporting of Incidents and Crimes
The purpose of this Order is to standardize procedures for the reporting of incidents and
crimes that occur at the Main Jail and the Rio Cosumnes Correctional Center
(RCCC).
I. Policy
Custody staff shall document all incidents that result in physi cal harm, or serious threat
of physical harm, to an employee , prisoner or other person with in 24 hours of the
incident. Specified documents shall be maintained in the admin istrative area of the
division and/or with the Records Bureau.
II. Reporting Criteria
A. California Code o f Regulations Title 15, Article 4, Section 1044, requires
documentation and maintenance of w ritten records of all inciden ts that
result in physical harm, or seri ous threat of physical harm, to an employee,
inmate or other person.
B. In addition to the incidents listed above, staff shall prepa re written
documentation in accordance with the corresponding Operations O rders
when:
1. Force is used as per Operat ions Orders 2/01, Use of Force; 2 /05,
CERT and Cell Extractions; and 2/04, Weapons, Tactical Weapons,
and Specialized Equipment.
2. Restraint devices are used as per Operations Order 2/02, Use of
Restraint Devices.
3. The Prostraint Chair is used as per Operations Order 2/03, U se of
the Prostraint Chair.
4. A Safety Cell, Segregation Ce ll, North Holding #2, or Multip urpose
Rooms are used as per O perations Order 4/05.
C. The completed documentation s hall be compiled in an “Inciden t Report
Package.”

A. In the situations described in Section II of this order, doc umentation shall
be completed. The Incident Repor t Package documentation may co ntain
any combination of the following documentation:
1. Crime, Casualty or Inci dent/Information Report
2. Jail Inmate Management System ( JIMS) Incident Report (PF-10)
3. On-line Watch Summary Log 4. Custody Log forms 5. Inter-Departmental Correspondences (IDC) 6. Restraint Extension IDC 7. Prisoner Death or Escape/ Wrongful Release Binders
8. Use of Force Routing Sheet
B. The information contained in t he documentation shall include identifying
information from involved and witnessing parties, and a chronol ogical
synopsis of the incident and obser vations made by custody staff . In the
event a crime report is generat ed, indication of whether or not prosecution
is desired shall be included.
C. Completion of required repor ts shall be in accordance with t he Sheriff’s
Department Report Writing Manual and General Order 1/34, Report s.
D. Administrative staff at each facility will maintain records as described in
Section II of this order.
IV. Documentation Requirements
A. Custody staff is responsible for completing a JIMS Incident Report (PF-10)
for violations of facility rule s in accordance with Operations Order 7/03,
Discipline Plan.
B. Custody staff shall minimally complete a JIMS Incident Repor t (PF-10),
Watch Summary Log, and log book entry for ALL VIOLATIONS OF
CRIMINAL STATUTES that take place within the facilities whether or not
a victim desires prosecution.
1. A Crime Report shall be complet ed by sworn custody staff for
felony violations of criminal st atutes that take place within the
facility.
2. Misdemeanor or infraction crim es or incidents of a minor nat ure
may be handled administratively th rough the disciplinary proces s,

at the discretion of a Sworn Super visor, unless a victim desire s
prosecution.
3. Additional documentation as described in Section III may be
necessary.
C. In cases where a Crime Report has been co mpleted and an arre st has
been made, an administrat ive disciplinary process may continue separate
from any criminal proceedings.
D. If a Crime Report has been genera ted and the crime involved injury to a
person, the reporting officer sha ll attempt to obtain the victi m’s signature
on a “Release of Medical Informat ion” form. If the victim refu ses to sign,
the officer will write “refused” on the signatur e block of the medical release
form. Medical witness statement s providing diagnosis, prognosi s and
treatment received should also be included in the report, if po ssible.
E. A Casualty Report and Release of Medical Information form sh all be
completed in all situations wher e a prisoner is injured while i n the custody
of the Main Jail or RCCC .
1. Exception: If a Crime Report has been generated and the inj ury is
described in the crime report, a separate Casualty Report is no t
necessary. However, a “Release of Medical Information” form
signed by the victim and Medical witness statements providing
diagnosis, prognosis, and treatmen t received should be included in
the report, if possible.
