CRIME SCENE INVESTIGATI ON INFORMATION CARD
The purpose of this order is to establish procedures for the us e of the Crime Scene
Investigation Information Card. I. Request for Crime Scene Investigator
A. In all cases, if an officer must clear the scene prior to th e arrival of the
crime scene investigator, t he requesting officer shall:
1. Request a report number from Rec ords via telephone or Channe l 2
except in robbery or stolen vehicle cases in which the
Communications Section shall provide the report number.
2. Contact the dispatcher and r equest that a CSI Unit be dispat ched to
the scene and provide t he dispatcher with t he address, nature o f
the crime, and t he report number.
3. Complete a Crime Scene Invest igation Information Card (7462
Form 010) with all information nec essary for proper completion of
the CSI report and any particula r information about the scene w hich
may assist the CSI officer in c onducting his/her investigation.
Include the report number on the card .
4. Leave the card with the vict im, if present. Otherwise, place the card
in a location that can be eas ily found by the CSI officer. In
10851CVC cases where the vehicle is to be towed, leave the card
in the vehicle so that the CSI officer can find it.
II. Photographs – Misdemeanor Assault Victims/Composites
A. Whenever possible, officers handling cases requiring photogr aphs of
misdemeanor assault victims and victims/witnesses for a compos ite shall:
1. Direct the victims/witnesses to contact the Identification Bureau
between 0800 and 17 00 hours, Monday through Friday, for an
appointment to have injuries photographed, or to assist in ma king a
suspect composite.

2. Give the victims/witnesses a co mpleted Crime Scene Investiga tion
Information card (including report number) and instruct them t o
bring the card with them to the Identific ation Bureau.
