EQUIPMENT MALFUNCTION OR FAILURE
The purpose of this order is to establish the procedure for dea ling with equipment
malfunctions within the Communications Bureau.
I. Policy
A. The Communications Bureau is a maze of intricate, technical, and
mechanical equipment which must r emain operati onal on a 24-hour ,
seven-days-a-week basis. Both mi nor and major equipment malfunc tions
may occur. It shall be the respons ibility of the watch supervis or to evaluate
and determine the nature of the pr oblem and take the most exped itious
means of correction.
II. Minor Malfunctions or Failure
A. A minor malfunction or failure is one which does not immedia tely hinder or
deteriorate the safe, efficien t, and lawful operation of the Co mmunications
Bureau, and may easily be rectifi ed. Minor malfunctions or fail ures can be
corrected in one of two ways: 1) remedial repairs or mechanical
manipulation of controls may be made; or 2) temporary replaceme nt of
malfunctioning equipment or phy sical movement of personnel to a new
duty station pending repair.
III. Major Malfunctions or Failure
A. A major malfunction or failure is one which immediately hind ers or
deteriorates the safe, efficien t, lawful operation of the Commu nications
Bureau, and may not readily be re ctified. The watch supervisor confronted
with a major malfunction or failu re will take immediate action to correct the
problem: 1) make repairs, manipulat ion, and movement necessary to
temporarily restore operation; and 2) make immediate contact wi th the
designated Information Services Bureau representative so that r epairs
may be made.
IV. Reporting Malfunctions or Failures
A. Malfunctions or failures will be reported as follows:

1. For minor malfunctions (bus iness or non-business hours) the
Communications Bureau Request for Repair/Maintenance form
(see Appendix A) will be com pleted and forwarded to the
Information Services Bureau.
2. For major malfunctions the In formation Services Bureau will be
notified immediately.
3. In the event telephone repair is required, the Pacific Bell Telephone
Company has responsibility fo r maintenance and repair of
telephone equipment within the 9 11 communications and dispatch
areas. Requests for repair wil l be made through the 911 phone
repair service by calling 811-8081. County telecommunications h as
responsibility for maintenance an d repair of phone equipment wi thin
the Communications Bureau admi nistrative areas. Requests for
repair will be made through Co unty telecommunications.
4. County telecommunications is responsible for the maintenance and
repair of the Motorola 800 MHZ radio system. However, to facili tate
repairs in a timely manner, r equests for repair will be made th rough
the Information Services Bureau.
5. All 911 routing/display erro rs will be recorded on the prope r form.
Completed forms shall be forwar ded to the administrative assist ant.
