ID Bureau Page 1 of 2 2/21.1 (REV 8/78) CHANGE OF ADDRESS – CONVICTED SEX AND NARCOTIC REGISTRANTS The purpose of this order is to provide procedures for changing the address of a sex or narcotic registrant. I. Registrants Convicted sex or narcotic off ender registrants are required to notify the department of a change of address either in person or by mail. Identification Section personnel shall proce ss the change of address as follo ws: A. In Person: 1. Obtain registrant’s copy o f Permanent Registration receipt . 2. Require registrant to compl ete an information sheet. Check s heet to ensure completeness. Record near est cross street of residence as supplied by registrant. 3. Complete out-card and obtain registrant’s personal history f older from Records. 4. Take one black and white front al photo of registrant for per sonal history folder. 5. Complete the Department of Jus tice Change of Address form in duplicate. Require applicant to sign both forms. a. Send original to DOJ b. File duplicate in personal history folder. 6. Complete a new Permanent Regi stration form. Place applicant’ s right index fingerprint on form. Require registrant to sign th e form. Give completed registration form to registrant. 7. Forward a completed Master I ndex card to Records. Informatio n to include on the card: name, fi ngerprint number, date, and addre ss. Label card “ Sex or H&S Registrant – Change of Address”. Upon notification of a change of address, complete out-card and obtain registrant’s personal history folder from Records. 2. Complete a Department of Jus tice Change of Address form in duplicate. a. Forward original to DOJ b. Place duplicate, along with registrant’s letter of notific ation, i n t h e p e r s onal history folder. 3. Forward completed Master Index card to Records. Information to include on the card: name, fi ngerprint number, date, and addr ess. Label card “ Sex or H&S Registrant – Change of Address ”. 4. Return personal history folder to Records. II. Sex Registrant File Index cards are maintained on al l 290PC (sex offender) registra nts in the Identification Section. Pull t he alpha and area cards from the index file. The date and new address shall be recor ded on each card. Check map book for page and grid coordinates and place this information next to the address . Re-file the index cards.
