Daily Activity Report
The purpose of this order is to establish the procedures and re quirements for
completing the daily activity report. I. General
Officers shall complete a daily activity report (log sheet) dur ing each shift
worked, and shall fill in reques ted and pertinent information i nto the
appropriate sections of the repor t. The daily activity report records the
calls and other shift activity of patrol officers.
The required information to be entered at the top of the daily activity
report will include:
A. Officer’s last name and badge number.
B. Watch, unit number, area assigned
C. Date and day
D. Log on and off times
E. Vehicles and Mileage
F. Radio
G. Shotgun
H. Condition of equipment
1. Major problems with equipm ent shall be noted and written
up utilizing the proper forms and procedures.
II. Event information
A. Times
1. 51: the time the unit goes enr oute to the call. This may be
left blank if the officer initia tes the event and has already

arrived (i.e., vehicle stops, c ode-7, wavedowns, etcetera.)
2. 97: the arrival time of the unit.
3. 08: the clearing or in-service time.
B. How received
1. R: dispatched verbally through radio. 2. Tr/ov: initiated by the unit to radio (tr) or by on view (o v)
3. Mct: dispatched through communication terminal.
C. Type Category or type of call (459r , 211ip, 920j, and etcetera)
D. Location 1. Address: address where the event is occurring or has occur red.
2. Complainant: name of the co mplainant, reporting party or c ontact person.
3. Victim: name of the victim , if different from that of the complaining person.
E. Action 1. R: if a report was taken. Include report number in the app ropriate block.
2. S: if no report was taken (short form). 3. F: if unit was cleared or freed from call prior to arrivin g.
F. Disposition The officer’s disposition shoul d, when possible, contain a brie f summary of the
officer’s actions and names o f field contacts made. If a repor t was taken, the
report title and appropriate codes should be noted. If a vehic le was involved, the
description, license plate, appr opriate vehicle codes and any c itation number
should, when possible, be noted. If a unit was freed by the dispat cher or to respond to a higher priority call, the
officer will note the reason or ev ent number in the disposition for the freed call.
G. Report number The number(s) of any written report(s) taken.

A. Officers shall log all event s beginning with the first assi gnment of the shift
including all duties performed pr ior to patrol (i.e., briefing, report writing, and
etcetera) until end of watch. For example, the typical first l og entry for a dayshift
officer would read: time type locati on disposition

B. Officers shall log all dis patched and on view ed events duri ng the shift (i.e.,
600w, 10-10's, 10-7's, code 7's, and etcetera). Iv. Special entries
A. Any unusual problems with t he patrol vehicle that are found during the pre-
shift inspection or during t he shift shall be brought to the at tention of a patrol
sergeant. B. Any unusual circumstances, patrol requests or checks that t he officer
performs during the shift sha ll be noted on the log sheet.
V. Reporting responsibility
The patrol sergeant shall: A. Ensure that a completed daily activity report is submitted for each unit fielded.
B. Review and initial each daily activity report prior to the officer's departure at
end of watch. C. Ensure that the daily activ ity reports of the shift are for warded to patrol
clerical staff. D. Utilize the daily activit y report for purposes of feedback to the patrol officer as
deemed appropriate.
