Field Services Page 1 of 2 02/15 (REV 4/04) New Report Numbers The purpose of this order is to define the procedure to be foll owed in obtaining new report numbers prior to the repor t being submitted to a supervi sor for approval. I. Requirement All reports generated by personnel assigned to Field Services s hall have the report number written on the orig inal report before it is submi tted to a supervisor for approval. Officers will obtai n the new report number from either the Communications Bureau or Recor ds Bureau, depending on the type of call. II. Report Numbers Through Communications Officers will obtain new report num bers from the Communications Bureau by telephone under the following circumstances: A. Robbery - This ensures that Co mmunications obtains all the n ecessary information for a broadcast card. B. All Vehicle Reports - (Stolen , Stored, and etcetera) - This assures that the Communications Bureau obtains all the necessary information for entry into the CLETS and NCIC computers (see Patrol Operations Order "Stolen Vehicle Complaints"). C. Petty Thefts - Involvi ng the following thefts: 1. Vehicle Identificat ion Plates (VIN) 2. License plate registration tabs 3. License plates This ensures that Communications obtains the information necess ary for entry into the CLETS and NCIC com puters (see Patrol Operations Order "Stolen Vehicles"). Field Services Page 2 of 2 02/15 (REV 4/04) III. Report Numbers Through Records Officers will obtain new report num bers from the Records Bureau for reports other than those set forth above. IV. Special Circumstances Officers shall contact the Records Bureau by telephone for a re port number under any of the following situations: A. Rape or Sexual Assault B. Homicide C. Missing Persons - This assure s that the Records Bureau obtai ns all the necessary information for MUPS entry. Records will then supply the MUPS File Control Number (FCN) to be noted in the officer's rep ort D. Whenever it is advisable to keep the name of the Victim conf idential. V. Information Requirements In all cases, officers will be prepared to supply the following information to either the Communications or Records Bu reau for that particular type o f report. The information requested in shown in o rder of entry into the compu ter. A. Type of crime or incident B. Crime title (if applicable) C. Victims or reporting pers on's name and date of birth. D. Name and date of birth of any suspects arrested or cited. 1. Adult 2. Juvenile E. Location of crime or incident. F. Name and badge number of the o fficer writing the report
