Security Services Page 1 of 2 02/04 (REV 01/06) Log Book Entries The purpose of this Order is to o utline requirements for log bo ok entries. I. General Log book entries provide a method for permanently documenting a ctivities of Security Services Bureau per sonnel. They are discoverable b y court order and can be admitted in a c ourt proceeding. It is crucial that all log book entries be comprehensive and accurate. Log book entries a re a reference, providing accurate accounts as to what occurred duri ng past incidents. Nothing in this or der shall prevent Security Servic es Bureau personnel from entering any info rmation deemed necessary into a log book. II. Log Entries Required log book entries are as follows: A. Any incident for which our s ervices are either requested or required. B. Any officer self-initiated activity. C. Anytime an officer takes someone into custody. D. Anytime an officer issues a parking citation or parking warn ing notice. E. Anytime an officer completes a report. F. Anytime any an officer comp letes an application for a detent ion and a person is transported to the Sacr amento County Mental Health Treatment Center according to W elfare and Institutions Code 515 0. G. Any other information requi ring permanent documentation. II. Elements of a Log Entry As part of a proper log ent ry, personnel should: A. Document who was contacted. I nclude the names and dates of birth for victims, witnesses, and suspects. B. Document the circumstances of the incident. C. Document where the incident occurred. Security Services Page 2 of 2 02/04 (REV 01/06) D. Document how the incident was handled and the final disposit ion. E. Document Security Services B ureau personnel involved and who wrote the log entry. III. Log Book Corrections All entries will be accurate and legible. If an error occurs, the officer entering the information shall do the following: A. Draw a thin line through the inaccurate information. B. Initial the error. C. Enter the correct information. D. No correction fluid or tape shall be used in any logbook.
