1/33 (NEW 3/07) Report Number Cancellation This purpose of this General Order is to establish a policy for the cancellation of report numbers that have been issued. I. Policy It is the policy of this Departm ent that report numbers must be canceled by the immediate supervisor, or higher r ank above the immediate superv isor, of the individual that requested the number. II. Procedure A. Supervisors must call the Records Bureau and request that th e report number be canceled. B. Contact a Records Supervisor . If none is available, leave a detailed message with the required informa tion along with your contact n umber. 1. Day Shift Supervisor 874-4688 2. Swing Shift Supervisor 874-5351 3. Graveyard Shift Supervisor 874-6077 C. Advise the Records Superviso r to cancel the report number an d state a reason for canceling. D. The Records Supervisor will determine if the report number c an be canceled. If there is property attached, or if documents have been received in Records, then the r eport number can not be canceled . The Records Supervisor will advise the requestor that the report nu mber can not be canceled. E. The Records Supervisor will enter a comment into RMS stating the reason for cancellation and the requesti ng supervisor’s name and badge number. F. The Records Supervisor will ca ncel the report and confirm wi th the supervisor that the cancellation is complete.
