ATTENDANCE RECORD DOCUMENTATION
The purpose of this order is to e stablish a method for document ing the attendance
record of all employees in the Communications Bureau.
I. Policy
It is the responsibility of the watch supervisor or their desig nee to ensure that
accurate and timely attendance records are maintained.
II. Attendance Record Form
A. The attendance record form is intended to be a supervisory t ool which
may be used for the following:
1. Provide an easily recognized attendance chart for evaluation
purposes.
2. Assist in providing a more equitable distribution of overtim e
opportunities.
3. An aid in determining watch/ days off assign ments and in appr oving
holiday time off requests.
III. Procedure
A. The attendance record form provides sufficient space to docu ment an
entire year’s attendance. Informat ion will be transcribed onto this form
from the various watch schedules at the end of each pay period by the
watch supervisor or their designee.
B. The following employee descrip tive data will be maintained o n each form:
1. Badge or radio number 2. Division 3. Date of rank 4. Date of hire

C. The leave abbreviations to be used are included at the botto m of the form.
All absences with the exception of sick leave and family sick l eave will be
recorded in black ink. Sick leav e categories will be noted in r ed ink.
D. Overtime work will also be indicated.
E. All forms will be stored in the supervisor’s office in a spa ce designated by
the watch supervisor. They wil l be readily available to supervi sors at all
times.
F. At the end of the calendar year the completed attendance rec ord form will
be filed in the employee’s division personnel file.
