Subject: Supporting Paperwork Required When Writing a Case Report
The following outline indicates the necessary supporting paperwork required when writing a case
report for submission to Report Review Unit. Sworn statements are required from any and all
victims and witnesses to include deputies that are victims of Battery . Verify the incident report
is signed and notarized in the appropriate blocks as this will constitute sworn testimony from the
deputy.
1. Felony Arrest - Adult
A. Incident Report - Page 1 (All Incident Reports will be reviewed and
approved by a supervisor before being sent to the State Attorney ).
B. Incident Report - Page 2
C. Arrest Affidavit or Affidavit for Arrest Warrant with the Arrest Warrant
D. Property Form (when required for evidence)
E. Sworn Statements (when required)
F. Report Routing & Affidavit Form (Jurat)
2. Felony - Filing (Adult)
A. Incident Report - Page 1 (All Incident Reports will be reviewed and
approved by a supervisor before being sent to t he State Attorney ).
B. Incident Report - Page 2
C. Arrest Affidavit (at large)
D. Property Form
E. Sworn Statements (when required)
F. Report Routing & Affidavit Form (Jurat)
NOTE : Each defendant's arrest affidavit must have its own narrative.
3. Misdemeanor Arrest – Adult
A. Incident Report - Page 1 (All Incident Reports will be reviewed and
approved by a supervisor before being sent to the State Attorney ).

B. Incident Report – Page 2 (When the arrest affidavit is used to report a
misdemeanor arrest, only Page 1 of the Incident Report Form will be
completed.
C. Arrest Affidavit - normally serves as Page 2 narrative of the report
D. Property Form (when required for evidence)
E. Sworn Statements (when required)
F. Exception to A. and B. above, misdemeanor vice related arrests by
Vice/Narcotic Agents relating to prostitution and the illegal sale, giving,
serving of alcoholic beverages to persons under age twenty -one (21).
G. Report Routing & Affidavit Form (Jurat) – only required if an Incident Report
Page 2 is completed.
NOTE : Each defendant's arrest affidavit must have its own narrative.
4. Misdemeanor - Filing (Adult)
A. Incident Report - Page 1 (All Incident Reports will be reviewed and
approved by a supervisor before being sent to the State Attorney ).
B. Incident Report - Page 2
C. Property Form (when required for evidence)
D. Referral Notice (if victim is unsure about prosecution)
E. Sworn Statement (if victim desires prosecution)
F. Report Routing & Affidavit Form (Jurat)
5. Traffic Arrest - Felony DWLS, Habitual Traffic Offender (HTO) and Fleeing and
Attempting to Elude
A. Incident Report - Page 1 (All Incident Reports will be reviewed and
approved by a supervisor before being sent to the State Attorney).

C. Arrest Affidavit
D. Sworn Statements (when required)
E. Traffic Citations - make a photo copy of the criminal uniform traffic citation(s)
and forward to the Traffic Enforcement Section. The deputy shall leave the
original white copy of the criminal uniform traffic citation(s) with the arrest

affidavit at the Booking Receiving Facility or Juvenile Assessment Center.
In all felony traffic a rrests, the arresting deputy shall make a second copy
of the uniform traffic citation used to document the felony traffic crime and
attach this copy of the citation to the arrest packet being sent to Report
Review.
F. Property Form (when required for ev idence)
If you do not issue a citation for the offense for which you first stopped the
offender (i.e., expired tag, broken taillight, etc.,) list on the front of the
criminal citation the reason for the original stop.
G. Report Routing & Affidavit Form (Jurat)
6. Juvenile Felony Arrest
A. Incident Report - Page 1 (All Incident Reports will be reviewed and
approved by a supervisor before being sent to the State Attorney ).

D. Sworn Statements (when required)

7. Juvenile Felony At Large and/or Release Agreement Cases
A. Incident Report - Page 1 (All Incident Reports will be reviewed and
approved by a supervisor before being sent to the State Attorney ).
B. Incident Report - Page 2
C. Arrest Affidavit – At Large (Original signatures required on Clerk of Court
copy).
D. Sworn Statements (when required)
E. Property Form (when required for evidence)
