This order consists of the following:

1. Purpose
The purpose of this policy is to accurately document the activities of agency personnel in
enforcement related activities.
2. Policy
It is the policy of the Sheriff's Office to document incidents in an efficient manner.
Documentation shall preserve the reputation and integrity of the agency and improve th e
overall rate of corrections. Efficient documentation includes use of technology to complete
agency forms when possible.
3. Procedures
A. Personnel shall write Incident Reports for all verified criminal acts reported. An
Incident Report may be written for any call for service at the discretion of personnel
handling such calls. Superv isors may require reports be written by subordinates
for any type of incident.
B. Communications personnel shall assign a unique computer generated number (an
event number) to each call for service received, whether by telephone or by radio
request from f ield personnel. Only those reports that are filed in Records shall be
assigned a case number.
C. Supervisors of personnel writing reports shall review reports daily to verify
accuracy and detail, and to prevent or control delinquent reports. Supervisors shall
confirm reports have been written for all calls -for-service requiring reports.
Supervisors shall confirm reports are completed in compliance with agency
directives and shall sign each repor t prepared by subordinates. Reports shall be
turned in prior to the end of each shift unless a supervisor approves otherwise .
D. All agency incident and supplemental reports and charging affidavits shall be
completed using the agency’s electronic automate d reporting tool. If this tool is
not available, a pa per report shall be completed. Other agency forms that are in
electronic form shall be utilized when practical.

F. Completing Field Reporting Forms
1. Incident Report Form
When completing an electronic incident report, the Incident Report Jurat
Form shall be completed as the oath for the report. The Incident Report is
to be used to document:

a. All reported offenses whether felony or misdemeanor.
b. Miscellaneous, suspiciou s and service incidents at the discretion of
the employee or his/her supervisor.
c. Exception, misdemeanor vice related arrests by narcotics agents
relating to prostitution and the illegal sale, giving, serving of alcoholic
beverages to pe rsons under age twenty -one (21).
d. If the victim of the crime is a tourist. A victim of a crime is considered
a tourist if they reside outside the six contiguous counties of Brevard,
Lake, Osceola, Polk, Volusia and Seminole.
2. Supplemental Report Fo rm
The Supplemental Report Form shall be used:
a. As a continuation of the narrative of an Incident Report when multiple
pages are required or when more than one (1) employee is required
to write a report on the same incident.
b. To record additional infor mation from a previously reported offense
or incident.
c. Employees shall check the appropriate block at the top of each
Supplemental Report to reflect whether the report is a continuation
of the original or additional information.
d. When completing an el ectronic incident report, the Report Routing &
Affidavit Form shall be completed as the oath for the report.
3. Charging Affidavit Form
a. The Charging Affidavit Form shall be used to document each arrest.
