This order consists of the following:

1. Purpose
The purpose of this policy is to verify that agency members appropriately report changes
necessary to maintain accurate re cords for insurance, compensation, and notification
purposes.
2. Policy
Employees are required to immediately notify the agency anytime there is a change in their
personal information. Full -time and part -time employees (excluding School Crossing
Guards) must update address, telephone number, and emergency contact information in
the Lawson database. All School Crossing Guards and volunteers must notify the agency
in writing of all changes related to name, address, telephone number, emergency contact
information , or other relevant information.
3. Procedures
A. Full-time and part -time employees must immediately update any change in address,
telephone number, or emergency contact information in the Lawson database. If
the employee’s name, number of dependen ts or marital status changes the
employee must notify Human Resources within thirty (30) days of this change.
Information on making changes to benefits is found in GO 4.3.0 .
B. To assist in preparing promotional resume, the Lawson training database, salary
incentive requests and duty assignments, employees who complet e training courses
or educational programs must forward copies of certificates of completion or
attendance to Human Resources and Training. Failure to forward certificates may
prevent an employee from attending any further schools/seminars until this
requirement has been fulfilled.
