Evidence Tech Processing Request Form The purpose of this order is to set forth Department procedure regarding the use of the Evidence Tech Processing Request Form (TF -3389(02/16)) PROTOCOL The purpose of this form is to facilitate the com munication of information between Investigators in the Criminal Investigation Division (CID) and Police Evidence Technicians (PETs) in Patrol regard ing evidence processing requests. The information requested in the form is the minimum information a PET re quires in order to fully process the evidence in question. The CID Investigator shall complete the form by providing the information requested in the fill -in boxes. The CID Investigator shall then submit the completed form via email to the assigned PET . If the PET is unknown, the form shall be submitted to the general PET email address : opdpet@oaklandnet.com . Any additional documents such as signed search warrants or signed consent forms should be attached to the email message and provided with the completed Evidence Tech Processing Form. The CID Investigator shall also contact Communications and advise of the processing request so it can be placed on the TEC CAD screen. PROCEDURE 1. Provide all the requested i nformation, if applicable to the request, in the provided fill-in box es. The box es will expand to allow as much information as needed. 2. If there is more than one victim or suspect, please use the “Additional Victim(s) ” and “Additional Suspect(s) ” boxes to add the information. a. This will assist the PET in looking for additional evidence such as indicia that may be in the vehicle being processed that is not in the primary victim or suspect name. b. If there are no additional persons the boxes can be left blank. 3. Indicate if the vehicle requested to be processed belongs to the victim or suspect by filling in the box: “Victim or suspect vehicle” as either “victim” or “suspect” 4. If a search warrant has been sought and signed by a judge indicate “yes” in the appropriate box and attach a copy of the warrant. 5. Provide as much information as possible for “Specific Evidence Items of Interest” or “other comments/requests” so the PET will know if there is a specific item that TRAINING BULLETIN Evaluation Coordinator: CID Com mander Automatic Revision Cycle: 3 Years “Department Training Bulletins shall be used to advise members of current police techniques and procedures and shall constitute official policy.” Index Number: II-G Alpha Index: Evidence Tech Processing Request Form Effective Date: 21 Mar 16 2 Sequential Double Blind Photographic Line-ups, Index I-T is expect to be located or if there is a special re quest that was not answered in the earlier questions that may need additional focus and/or attention by the PET. 6. Include CID I nvestigator name and contact number . The PET will use this information in case of additional questions or to make notifications s uch as the complet ion of a proce ssed v ehicle. When the form is completed , the CID Investigator should save a copy for the case packet and email a copy to the assigned PET or to the General PET email: opdpet@oakl andnet.com . The general email will be checked during all shifts and an available PET will self-assign the task and notify the investigator that the request is assigned. The CID Investigator shall contact Communications and advise of the processing reque st so it can be placed on the TEC CAD screen (otherwise known as “stacking the request”) and indicate that the request form has been completed and forwarded to the Evidence Tech Detail. When stacking the call, relevant information such as RD (report) numbe r, name of investigator, and type of request must provided to Communications in order to link the stacked call to the emailed request form.
