MOUNTAIN VIEW POLICE DEPARTMENT SPECIAL SERVICES SECTION TRAINING AND INFORMATION BULLETIN PAGE 1 OF 16 2023 DATE: September 13, 2023 BULLETIN NUMBER: 23-10 SUBJECT: Axon Standards Use of Force Completion INFORMATION: To assist Officers with Use of Force report writing and to comply with URSUS requirements, Officers (with the assistance of Sergeants) or Sergeants will be completing Use of Force reports in AXON Standards. This document is a step -by-step guide for Officers and Sergeants on completing this form in AXON Standards. Other forms to be completed in AXON Standards include 24 Hour Report, Awards and Commendations, Citizen Complaint , Internal Complaints , Vehicle Collision and Vehicle Pursuits. Step 1: Logging into Axon Standards Log on to Evidence.com using your existing account and select “ Standards.” 2 of 16 Step 2: Creating a new event. In Standards you will see “My Task” which will be any reports in progress for Officers and reports for approval or in progress reports for Supervisors. Select “New Event” to create a new Form. 3 of 16 Step 3: Selecting appropriate form. Select “Use of Force” and Start Event. Officers only option for creating an event is the Use of Force Form. 4 of 16 Step 4: Creating event information. Complete the Event Information page. 5 of 16 Step 5: Adding citizen. Complete the “Citizen Page.” If you believe the citizen has a profile in Standards select “known Person” and search. If know to Standards their information will auto populate. 6 of 16 Step 5 Continued: If know , their information will show at the bottom of the screen. Select the most current/accurate information. Step 5 Continued: If profile is correct, select Import & Update . Make sure the profile is updated to the most recent information. If unknown all pertinent/known fields will need to be manually entered . 7 of 16 Step 6: Adding Officer information. Select “Add Officer” and if the officer used force select “I – Involved” in the “ROLE” box. 8 of 16 Step 6: Continued: After “I -Involved’ is selected “ADD FORCE” will appear at the bottom of the screen. 9 of 16 Step 6: Continued. Either the officer , or the Sergeant with the assistance of the officer , will need to complete step 6 to accurately obtain the information pertaining to the “perceived” questions. Questions are also written in a manner to assist the office with report writing and jog the officer’s memory of what should be included in a police report. After selecting “Add Force” complete all boxes to include “Force Type.” 10 of 16 Step 6 Continued: Depending on the “Force Type” selected appropriate boxes will show depending on what is selected. (e.g., if Taser is selected “Warning Given” “Clothing type” “Effectiveness” etc. will show). If an Officer is a witness only the “Add Force” option will not appear. 11 of 16 Step 7: Investigation Details. No narrative is required if attaching Police Report 12 of 16 Step 7: Attach Files Select “Files” then “Upload File .” Step 7 continued: If the file is already in Evidence.com it can be located by selecting search and searching for the existing file. 13 of 16 Step 7 continued: For files not already in Evidence.com (e.g Police Reports and Audio copy of the radio traffic) you will first need to save them. Then select “upload” then “Select File” and select the appropriate file. 14 of 16 Step 8: Review If an officer is completing the Axon Standards Report the review tab will not appear. The officer will complete the portions they can complete (e.g officer and citizen tab) and click “Submit” 15 of 16 Step 8 continued: The officer will select “Chain of Command Review” and assign to the appropriate Sergeant. Step 8 Continued: If Sergeant is completing the report, then first select the “Review” tab then select “Add Reviewer.” 16 of 16 Step 8 Continued: Complete all tabs and indicate if within policy. This tab has replaced the previous Use of Force Memo. Then complete “Chain of Command Review” to the appropriate Lieutenant. Then complete “Chain of Command Review” to the appropriate Lieutenant.
