SUBJECT: Evidence Processing Requests on the DOC
INFORMATION:
The following information is intended to provide guidelines for submitting evidence
proce ssing requests via the DOC. Submitting requests via the DOC allows them to be
assigned to the appropriate investigator, investigated in a timely manner, and
appropriately tracked.
1. When submitting a request, begin by clicking on the Evidence Processing tab at the top of the DOC home page after you sign in
2. You will then be prompted to choose from three (3) different options depending
on the type of evidence processing you are requesting (Cell Phones,
Computer/Media or General Evidence). *Please only submit requests for video
surveillance retrieval if you are unable to do it yourself during your normal
work hours*

3. After clicking on one of the options, it will then take you to a separate page with
several areas to complete, in cluding the information specific to your case

4. Please remember the following important steps/information regardin g the
submission process:
• In the portion where you attach the associated search warrant (PDF) for your
case, Please only submit the search w arrant , NO AFFIDAVITS
• You MUST submit an evidence processing request for each item you are
requesting (1 item = 1 request). Tracking requires each item to have an
individual request submission
• There is a place to put your information at the bottom (N ame, Email address and
Phone number). Under the phone numb er portion, it is going to show up as
encrypted
• Please do not put any personal information in these areas
5. Once requests are submitted, they will be assigned to an investigator who will
comm unicate with you directly
• Reminder: Items to be sent to the crime laboratory for analysis (Other than
controlled substances) need to be submitted via the DOC as well, and then they
will be entered into the count y lab tracking system and taken for processing by
MVPD Evidence.
