I. The Department’s Stolen Vehicle Affidavit form will be prepared in conjunction
with other required reports for all reported motor vehicle thefts. The Stolen
Vehicle Affidavit is to be signed by the victim or the person reporting the vehicle
theft. The Stolen Vehicle Affidavit will be attached to and submitted with the ORI.
The officer taking the report will sign and date the Stolen Vehicle Affidavit upon
completion.
