I. Incidents/Occurrences Requiring an After-Action Report [41.2.2k; 41.2.3e; 46.1.2; 46.1.3i] A. An After-Action Report will be completed for the below listed incidents/occurrences: 1. Man-made and natural disasters; 2. Major fires; 3. Major aircraft accidents; 4. Accidents involving hazardous materials; 5. Collapsed buildings; 6. Hurricanes and/or Tornadoes; 7. Civil disturbances; 8. SWAT operations and/or hostages and/or barricades subject situations; 9. Mass arrests; 10. Bombings threats/explosions when mutual aid is requested; 11. Vehicle pursuits; 12. Special events; 13. Terrorist acts; 14. Active threats to include mobilization of additional Department personnel necessary to address the active threat; 15. Unusual occurrences; and 16. Requests for police response to major incidents in other jurisdictions. B. The After-Action Report will be prepared by the supervisor responsible for the incident. The report will be prepared within 10 days after the culmination of the event unless otherwise directed by the Chief of Police. C. An After-Action Report does not relieve any supervisor from preparing the Special Incident Notification (SIN).
