KISSIMMEE POLICE DEPARTMENT GENERAL ORDER General Order 214 – Crash and Incident Review Process Page 1 of 2 TITLE: CRASH AND INCIDENT REVIEW PROCESS 214 EFFECTIVE: 07/01/2015 REVISED: 09/26/2025 SCOPE : ALL EMPLOYEES REVIEW AUTHORITY: PROFESSIONAL STANDARDS SECTION LIEUTENANT CFA STANDARDS: 14.03, 15.15M (A-B) 214.01 General Provisions 214.04 External Reporting Requirements 214.02 Supervisor Responsibility 214.05 Internal Reporting Requirements 214.03 PSS Lieutenant Responsibilities Policy: The City of Kissimmee and Kissimmee Police Department have an established review process to identify trends involving work related injuries, vehicle crashes, and damage to loss of Department property. This standardized process ensures consistency and timeliness in the review of crashes and incidents. 214.01 General Provisions: Supervisors shall be responsible for reviewing the actio ns of their employees and taking the appropriate corrective actions. Employees shall not be disciplined for work related injuries unless an unsafe or negligent action contributed to the injury. Employees shall not be disciplined for damage to Department pr operty unless an unsafe or negligent action contributed to the incident . An employee shall immediately notify a supervisor if they are involved in a crash with the Department vehicle. An employee shall immediately notify a supervisor if they have been injured due to a work -related incident. The supervisor shall confirm the appropriate reports are completed to document the cra sh or incident. The completed reports shall be forwarded to an on -duty supervisor. If the crash or incident was recorded, the video shall be obtained and entered into evidence. [CFA 15.15M – A] 214.02 Supervisor Responsibility: A supervisor shall respond to the scene when an employee is injured, involved in a crash, or has an incident involving damaged or lost Department property. [CFA 14.03] The City of Kissimmee Incident Report and/or City of Kissimmee Employee Workers Compen sation Notice of Injury Report shall be completed as applicable. Photos of the injuries, scene or damage shall be attached to the report as necessary. The appropriate reports shall be obtained from any outside agency involved in the investigation. The supe rvisor shall also complete the City of Kissimmee Supervisor’s Accident Investigation Report. If the employee is found to be at fault or an unsafe or negligent action contributed to the incident, an Initial Notice of Inquiry KISSIMMEE POLICE DEPARTMENT GENERAL ORDER General Order 214 – Crash and Incident Review Process Page 2 of 2 shall be initiated. The original reports and related documents shall be forwarded to the employee’s Division Captain via the chain of command. [CFA 15.15M – A, B] The Division Captain shall forward the original reports and any related documents to the Professional Standards Section Lie utenant. The Professional Standards Section Lieutenant shall ensure a copy of the reports and any related documents are forwarded to the Risk Management Office if applicable. [CFA 15.15M – A] 214.03 Professional Standards Section Lieutenant Responsibiliti es: The Professional Standards Section Lieutenant shall review the paperwork associated with a work -related injury, an employee involved crash or an incident involving damage, of Department property. The Professional Standards Section Lieutenant shall mai ntain a log of crashes, employee injuries and incidents to identify potential policy failures, training issues or identifiable trends. Follow -up actions shall be taken as appropriate. 214.04 External Reporting Requirements: For purposes of this policy, the reporting stipulations on lost or damaged property shall coincide with General Order 307 – Report Writing and apply only if: a. The damaged or lost property is valued in excess of two hundred dollars ( $250 ); b. The damaged or lost property is a Department issued telephone; c. The damaged or lost property may be used to impersonate any employee; and d. The damaged or lost property may compromise the security of the police building. 214.05 Internal Reporting Requirem ents: For incidents not meeting the external reporting requirements; a City of Kissimmee Supervisor’s Accident Investigation Report shall be completed and forwarded through the Chain of Com mand. This report will be maintained by the Professional Standards Section Lieutenant. A replacement for the lost or damaged property may then be obtained through the Quartermaster’s office by the approved requisition process .
