SUBJECT: IDENTITY THEFT PROCEDURES
I. PURPOSE : The purpose of this standard operating procedure is to establish guidelines and
procedures for the correction of arrest records without a Court Order as they relate to Identity
Theft issues.
* II. SCOPE : This procedure shall apply to members of the Identification Admi nistration Unit and
Fingerprint Section .
III. PROCEDURE :
* A. Compromised Identity Review Claim Form initiated by Sheriff’s Office personnel .
1. If there is a clear case of identity theft without extenuating circumstances that
may require further inves tigation , have the individual complete the Florida
Department of Law Enforcement ( FDLE ) Compromised Identity Review Claim
Form . By completing this form, FDLE will notify other agencies that may have
incidents associated with the stolen information.
2. Fingerprint the individual on a Sheriff’s Office fingerprint card .
3. Once the FDLE Compromised Identity Review Claim Form is complete, verify the
name with the subject’s driver license or other form of identification .
4. Provide a c opy of the Compromised Id entity Review Claim Form and all other
documents to the individual . Mail the completed original Compromised Identity
Review Claim Form , fingerprint card , and any additional documents the individual
provides , to FDLE.
5. FDLE will return the completed inve stigation to the contributing agency, as well
as any other agency listing the identity theft information .
6. Once the completed investigation and Compromised Identity Review Claim Form
are returned from FDLE , and based on the supportive documentation in the
investigation , send copies of the correction letter to all affected areas to remove
all compromised information contained in the agency’s records .
B. Compromised Identity Review Claim Form initiated from another agency or state.
1. If FDLE sends the Sheriff’s Office a completed packet (to correct an arrest
record) initiated from another county or state, based on the supportive
documentation in the investigation , send copies to all affected areas. FDLE will
notify all other agencies affected to make necessary corrections to their records.
