Public Transportation Vehicle Driver Permit
502.1 PURPOSE AND SCOPE
The purpose of this policy is to establish a designation of procedure and responsibility for public
transportation vehicle driver permit issuance. A Driver Permit is required for the operation of a
public transportation vehicle upon the streets of the city unless waived by the Chief of Police.
(Section 5.80.020 Fremont Municipal Code).
502.2 PROCEDURES
The following is a chronological outline of the procedure for the issuance of public transportation
vehicle driver permits, and the organizational unit responsible for each step.
(a)Applicant for a Public Transportation Vehicle Driver Permit obtains forms from the front
counter at the Police Department. The Applicant fills out the forms, and obtains all
of the required items. The application package to be submitted by the applicant shall
consist of the following:
1.A completed and signed Taxi Driver Permit Application (sample in APPENDIX
503).
2.Two recent passport-type photographs of the applicant. The image of the head
shall not be less than one inch (1"), nor more than one and one half inches (1
1/2") from the top of the head to the bottom of the chin.
3.A copy of the completed Request for Live Scan Service form for fingerprints,
as required by the Department of Justice (DOJ). A list of companies offering
Live Scan fingerprint services is available at the Police Department front
counter. A completed City of Fremont Public Transportation Vehicle Driver
Permit Application.
4.A letter from the employing/contracting taxicab company, on company
letterhead, to the Police Department endorsing the applicant, or a completed
Intent to Employ/Contract Endorsement Letter form (sample in APPENDIX 503).
The letter shall include the name and date of birth of the applicant and signature
of an authorized representative of the company.
5.Funds in the amount required by the current City of Fremont Master Fee
Schedule in a check or money order made payable to the City of Fremont. NOTE:
The fee is non-refundable in the event the driver permit is denied.
6.Proof of a current drug test (within the last 30 days).
7.A current copy of the applicant's California Drivers License record from the
Department of Motor Vehicles (DMV).
(b)The package of materials is to be mailed to the Taxi Program Coordinator at the
Traffic Unit of the Fremont Police Department, P.O. Box 5007, Fremont, CA 94537,

or submitted at the front counter of the Fremont Police Department. After being
completed by the CSO, the applicant package is to be sent to the Operations Support
Specialist, Administration/Business Services, for processing of the fingerprints by the
California Department of Justice (DOJ).
(c)Upon receipt by the Fremont Police Department, the package of materials will be sent
to the Community Service Officer (CSO) assigned as the Taxi Program Coordinator
in Traffic. The package will be reviewed by the CSO and a file opened. A background
investigation will be conducted, which will include a check of the applicant's drivers
license, an inquiry of local records, the "wanted persons" and CRIMS systems. A
copy of the DOJ applicant fingerprint response via the Administrative Assistant to the
Captain of Administrative Services will also be reviewed and included in the file.
(d)If there is any questionable history resulting from the background check or the DOJ
response, the Traffic CSO will submit the file to the Special Investigator to the Chief for
a review and decision as to whether or not the applicant will be allowed to continue the
permit process. The Traffic CSO will send appropriate letters to the company and the
applicant in the event the applicant is denied a permit. The letter denying the permit will
include an appeal process. A permit will not be issued, and all application materials,
including photographs and fingerprint cards, will be retained in the file until the file may
be purged as prescribed by law.
(e)If the driver's application is approved, the Traffic CSO will contact the applicant and
schedule a date for the written taxi driver tests. The Traffic CSO will administer all
three (3) tests consisting of a multiple choice test with an essay question, a location
map test, and an orientation test. The applicant shall be required to pass the written
examinations with a score of not less than seventy-five percent. If the applicant fails
any of the written tests, he/she may retake each test one additional time within thirty
(30) days. A second failure of any written test will result in the applicant being rejected.
He/she must then wait a minimum of ninety (90) days before reapplying for a permit.
(f)If the applicant passes all of the written tests, they will then be scheduled for a practical
driving test. This test will be administered by the Semi-Permanent Traffic Officer,
riding as a passenger in the cab. The applicant will be required to drive to two (2)
predetermined locations within the City of Fremont and will be evaluated on their
general driving skills, ability to find the requested locations in a timely manner (using
the most direct routes), ability to maneuver the taxicab, and familiarity with the taxi
meter. The officer will fill out an evaluation form and return it to the Traffic CSO with
a pass or fail result. If the applicant fails this test, he/she may retake the test one
additional time. A second failure will result in the applicant being rejected
