Traffic crashes can be reported by the use of two form s com monly referred to as the “Long Form ” and the “Short Form .” A Long Form Report (HSMV 90003) in its en tirety includes the Narrative/Diagram report (HSMV 90005) and any necessary Update/Conti nuation Report form (HSMV 90004) when the following criteria are m et: ¾ Death or personal injury, ¾ Leaving the scene involving dam age to attended vehicles or property (F.S. 316.061 (1)), ¾ Driving while under the influence (F.S. 316.193), The Short Form Report (HSMV 90006) is used to report other types of traffic crashes to the Departm ent: a driver report if the driver is required to file a written report to the Departm ent or as a driver exchange of inform ation form . If form HSMV 90006 is used as a Short Form , the officer who investigates the traf fic crash is only required to f ill in the shaded areas. However, law enforcem ent agencies can require their officers to fill in additional inform ation. The Florida Traf fic Crash Reports are com pleted by f illing in the blanks with required inform ation obtained from an investigation of the ev ent. In som e cases, the report displays a list of values for certain data that pertains to vehicles, drivers, pedestrians, passengers, and the scene of the traffic crash. The investigating officer is requi red to select and enter a value in the appropriate data field. Som e data fields are constructed to accept m ore than one value if warranted. The values needed to com plete the vehicle or pedestrian sections on form s HSMV 90003 (Long Form ), HSMV 90004 (Update/Continuation), and passenge r inform ation on form HSMV 90005 (Narrative/Diagram ) are located on the Long Form at the bottom of page one. The values needed to 2 complete the events section of these form s are displa yed next to the data fields on the back of form s HSMV 90003 and HSMV 90004. The rem aining data fields are com pleted based on the inform ation requested at the top of each category. It is im portant that those who investigate a nd com plete crash reports are doing so uniform ly to ensure accuracy of the data captured from the report and the tim ely subm ission of the report to HSMV within 10 days of com pleting the investigation as required by F.S. 316.066. 3 FLORIDA TRAFFIC CRASH REPORT LONG FORM HSMV 90003 and HSMV 90005 4 Time and Location Information (HSMV 90003) DATE OF CRASH DATE OF CRASH 01 30 06 Enter the date of the traffic crash in m onth, day, and year order in the following m anner: ¾ Display the m onth by using the num bers 01 through 12. ¾ Display the day by using the num bers 01 through 31. ¾ Display the appropriate year as required. TIME OF CRASH Enter the tim e of day or the approxim ate tim e of day the traffic crash occurred. ¾ Place an X in the AM or PM box. ¾ Midnight is considered AM and noon is considered PM. Use the 12-hour clock system to identify the tim e of the crash. Do NOT use the 24-hour clock system (a.k.a. m ilitary tim e).
