Unless the nonresident purchaser has completed a form prescribed by section 42-5009, subsection H, an affidavit in a form prescribed by the director shall accompany the application and shall contain the following statements: 1. The purchaser is not a resident of this state as defined in section 28-2001. For the purposes of this section and section 28-2154.01, the purchaser shall present to the department, an authorized third party or a motor vehicle dealer a driver license or other evidence prescribed by the director showing that the purchaser is not a resident of this state. 2. The vehicle is purchased to be registered out of state within ninety days after the issuance of the special ninety day nonresident registration permit. 3. The vehicle is not purchased for transfer to a resident of this state. 4. Other information that the director deems necessary. B.
